Immunotherapy & cell therapy

TIL therapy

Using immune cells from the tumor: tissue collection, manufacturing, conditioning and the demands of treatment.

Starting with cells already inside the tumor

Tumor-infiltrating lymphocytes are immune cells found within tumor tissue. TIL therapy obtains these cells from a surgically removed tumor sample, expands them in the laboratory and returns them to the patient after conditioning treatment. Unlike a conventional CAR-T product, the cells are not necessarily engineered with a single new receptor.

Two separate eligibility questions

A patient needs both a suitable tumor site for collection and sufficient fitness for the entire regimen. A safely removable sample, remaining measurable disease, previous treatments and organ function are assessed together. The ability to tolerate lymphodepleting chemotherapy and, for some regimens, interleukin-2 is as important as the tumor diagnosis.

What the melanoma approval actually covers

The US lifileucel indication is for adults with unresectable or metastatic melanoma previously treated with a PD-1-blocking antibody and, if BRAF V600-positive, a BRAF inhibitor with or without a MEK inhibitor. A melanoma diagnosis alone is insufficient. A different cell preparation or cancer type needs its own evidence and authorization.

Global treatment frontiers

Lifileucel’s US approval for previously treated advanced melanoma established a clinical route for TIL therapy in a solid tumor. Follow-up of the C-144-01 study, including a five-year analysis published in 2025, now helps assess how durable benefit can be for responders. The approval remains tied to the product and its full melanoma label.

The development question is how to extend benefit, improve manufacturing and reduce the burden of conditioning and supportive treatment. A TIL program for another tumor is a separate clinical investigation; the team should explain its evidence and protocol rather than treating the melanoma approval as validation of every TIL product.

Durable responses do not mean everyone benefits

C-144-01 was a single-arm study. Long follow-up helps describe response duration, but does not create a randomized comparison with another treatment. Ask whether an outcome is reported for all enrolled patients, infused patients or responders; excluding people who never reached infusion can change the impression of a treatment pathway.

A treatment episode, not just an infusion

Surgery and manufacturing precede the treatment admission. A patient may need other treatment while cells are prepared. Manufacturing may not yield a usable product, and clinical deterioration may prevent infusion. Ask how the team handles either situation and what costs are incurred at each stage.

The recovery plan should include infection precautions, blood-count checks, nutrition and a named clinician who can manage complications after returning home.

Four stages to plan before collection
StageWhat needs confirming
Tumor collectionA site that can safely provide sufficient tissue.
Manufacturing intervalDisease control, release criteria and a backup plan.
Treatment admissionConditioning, cell infusion and protocol-specific support.
RecoveryBlood counts, infections, organ function and local follow-up.

The whole regimen determines risk

Severe infections, prolonged cytopenias and cardiopulmonary or kidney complications may arise from the treatment sequence. Interleukin-2 can cause capillary leak and major blood-pressure changes. Treatment should be assessed as a package including surgery, conditioning, cell infusion and supportive medicines—not as a “natural” cell infusion without risk.

Being fit for surgery is not the same as being fit for TIL

Conditioning and interleukin-2 can create a substantial physiologic burden beyond the collection procedure. Review heart, lung and kidney reserve, active infection and recovery from earlier treatment. Before travel, obtain an explicit plan for deterioration during manufacturing and complications after discharge, including who can provide urgent care.

Treatment costs & individual consultation

TIL treatment costs require an individual assessment of the proposed product and available treatment pathway. Tissue collection, cell preparation, conditioning, infusion and supportive care all affect the overall cost.

Contact us for an individual cost consultation. Once a specialist team has assessed suitability and a feasible pathway, we can help clarify the relevant medical costs and follow-up arrangements. Travel and accommodation are separate.
