What is iza-bren, and what does EGFR × HER3 mean?
Iza-bren, izalontamab brengitecan, BL-B01D1 and BMS-986507 refer to the same drug. It is a bispecific antibody-drug conjugate (ADC): an antibody recognizes EGFR and HER3 and carries an anticancer payload.
EGFR is a target of the antibody; an EGFR mutation alone does not establish eligibility. The approved treatment pathway depends first on cancer type, pathology, recurrent or metastatic disease and previous treatment.
Who has an approved treatment pathway?
China approved the nasopharyngeal and esophageal squamous indications in June and July 2026. The separate requirements below follow the company’s disclosures of the registration certificates; they do not cover every head-and-neck, esophageal or EGFR/HER3-positive tumor.
| Cancer and setting | Previous treatment required | A distinction that matters |
|---|---|---|
| Recurrent/metastatic nasopharyngeal carcinoma | Failure of at least two lines of systemic chemotherapy and PD-1/PD-L1 inhibitor treatment. | Two chemotherapy lines do not mean two infusions. Radiotherapy alone or only one chemotherapy line does not establish eligibility. |
| Adult recurrent/metastatic esophageal squamous cell carcinoma | Failure of a PD-1/PD-L1 inhibitor combined with platinum-containing chemotherapy. | Squamous histology and the prior combination matter. Esophageal adenocarcinoma, chemotherapy alone or immunotherapy alone are not the same setting. |
What can be arranged now, and is a trial required?
For patients matching these indications, China has an approved-use pathway rather than access being limited to a trial. The company has reported commercial rollout. Chirona arranges specialist review through partner hospitals in China, then coordinates hospital drug supply, infusion appointments, tests and costs once a plan is agreed. The hospital and available dates are confirmed before travel.
What if I have lung or breast cancer?
There are clinical-development results in these cancers, and the company has disclosed acceptance of a triple-negative breast cancer marketing application. These are distinct from the two approved uses described here. We can coordinate specialist review of approved alternatives or specific research opportunities; recruitment and eligibility require separate confirmation. Chinese approval does not establish approval, supply or coverage in the US, Australia, New Zealand or Turkey.
What benefit has been shown in each cancer?
The randomized phase III studies address different patient groups. The published nasopharyngeal interim report primarily informs tumor response, while esophageal squamous results include a survival comparison. Results from one cancer should not be used to predict benefit in the other.
| Study and patients | Iza-bren versus chemotherapy | What this means |
|---|---|---|
| Nasopharyngeal cancer: 386 patients after at least two chemotherapy lines and PD-1/PD-L1 treatment; interim analysis published in The Lancet. | Objective response rate: 54.6% versus 27.0%. | More patients met the tumor-shrinkage criteria. Overall survival was immature in this report; response alone does not prove longer survival. |
| Esophageal squamous cancer: PANKU-Esophagus01, 497 patients after progression on immunotherapy plus platinum chemotherapy; sponsor-reported ASCO 2026 interim results. | Median overall survival: 9.8 versus 7.2 months; median progression-free survival: 4.2 versus 2.0 months. | Both endpoints improved in this study population. Medians do not predict an individual’s lifespan or compare the drug with every later-line option. |
Compare tolerability as well as tumor control
Low blood counts and infection risk can affect whether treatment can continue, and some adverse effects can be serious. Review tumor-control prospects alongside previous toxicity, nutrition, activity and reliable supportive care between infusions.
Which records and previous treatments need checking?
Use the relevant pathway below to organize your records. Drug names, treatment lines, combinations and reasons for stopping are more useful than simply saying you have had chemotherapy or immunotherapy.
Nasopharyngeal cancer: check chemotherapy lines and immunotherapy
- Confirm diagnosis and recurrence: send nasopharyngeal pathology, prior radiotherapy records and recent scans showing local recurrence, distant spread or both.
- List at least two systemic chemotherapy lines: include drugs, dates, response and reason for stopping, identifying platinum regimens. Multiple cycles of the same regimen generally remain one line, not one line per infusion.
- Specify PD-1/PD-L1 treatment: record the chemotherapy partner, any maintenance use, and progression or toxicity that ended treatment. A PD-L1 test result does not substitute for having received the therapy.
Esophageal squamous cancer: confirm histology and the prior combination
- Check the histology: the approved use described here is adult recurrent/metastatic esophageal squamous cell carcinoma, not automatically esophageal or gastroesophageal junction adenocarcinoma.
- Verify failure of the combination: provide records of PD-1/PD-L1 treatment combined with platinum chemotherapy and evidence of progression. Use of only one component, or separate use at different times, needs further review against current prescribing information.
- Address swallowing and nutrition: report food intake, weight changes, stents or feeding tubes, bleeding and radiotherapy. Significant swallowing difficulty needs nutritional planning alongside cancer treatment.
Do EGFR/HER3 tests or a previous ADC determine eligibility?
EGFR/HER3 targets do not replace the cancer, pathology and prior-treatment requirements. Do not arrange extra testing solely because these targets appear in the drug description. If you have had another ADC, provide its name, response, blood-count problems and any lung toxicity for the specialist’s comparison.
How is treatment given, and what needs monitoring?
Iza-bren is given by intravenous infusion. The prescribed dose, visits per cycle and need for admission depend on the treatment plan and your health. Plan infusions, blood tests and reassessment together; one infusion is not a complete treatment course.
Before and between infusions
- Before treatment: review blood counts, organ function and previous side effects. The treating team decides whether to proceed or adjust dosing.
- Between visits: watch for fever, marked weakness, bleeding, mouth soreness, nausea, vomiting and poor intake. Arrange anti-nausea care, mouth care and nutritional support.
- New cough or breathlessness: contact the team promptly to assess infection, disease changes or treatment-related lung injury. Severe breathing difficulty, substantial bleeding or altered awareness needs emergency care.
- Reassess on schedule: scans, symptoms and tolerability guide continuation. Report fever or a significant decline rather than waiting for the next appointment.
What could treatment cost, and why can the total change?
US$40,000–113,000
What’s included
This estimate covers total medical treatment costs, including medication, specialist consultations and assessment, pretreatment tests, routine laboratory and imaging follow-up, treatment administration and nursing, necessary hospitalization and monitoring, and routine supportive care. Travel and accommodation are not included.
Your individual treatment costs
Ongoing therapy does not have one fixed complete course for everyone. Weight-based dosing, infusions, tests, nutritional care and hospital needs differ between patients, and costs may exceed this range. We help obtain an itemized estimate for the drug, infusions, tests and routine support, and clarify charges for serious complications, ICU care or additional treatment. Coordination, travel and accommodation are confirmed separately.
How does Chirona turn a review into a care plan?
Start with your existing records. We organize them and arrange the relevant specialist review so you can understand eligibility, missing information, the treating hospital and monitoring arrangements before travel.
1. Send pathology and a line-by-line treatment history
For nasopharyngeal cancer, include at least two chemotherapy lines, PD-1/PD-L1 therapy and radiotherapy records. For esophageal squamous cancer, include pathology and immunotherapy-plus-platinum treatment. Both need recent scans, blood tests, current medicines and serious side-effect records. We identify gaps for the clinician to decide on further tests.
2. Confirm the hospital, drug supply, visits and costs
After the specialist agrees a plan, we coordinate partner-hospital appointments, drug availability, the first infusion, follow-up, costs and travel. If the history does not fit the approved pathway, the specialist first compares alternatives or research opportunities; booking a consultation does not establish treatment eligibility.
3. Arrange care between visits and after returning home
Confirm blood-test and scan schedules, swallowing and nutrition support, and urgent contacts for fever or breathing symptoms. We help prepare treatment and test records for your home clinicians. Later infusions elsewhere require a fresh check of local supply and clinical arrangements.
