Cancer care abroad. Know what your insurance covers.
Insurance for planned cancer treatment abroad depends on your policy, your treatment and the hospital you choose. Confirm all three before committing to travel—and keep a separate plan for costs you may need to pay yourself.
Sources & editorial approachStart with the kind of cover you have
“International” does not always mean that planned treatment is included. Read the policy’s benefits, exclusions and geographical limits together.
| Type of cover | What to establish |
|---|---|
| Travel medical insurance | Usually aimed at unexpected illness or injury on a trip. Do not assume it pays for cancer treatment that is the reason for travelling. |
| International health insurance | May include eligible planned care across a defined region. Check cancer benefits, underwriting terms, provider restrictions and prior approval. |
| Domestic or employer health cover | Ask whether planned overseas care is included, requires a referral or is limited to specific networks. An employer’s group contract can differ from an individual plan. |
| Evacuation & travel benefits | Medical transport, trip cancellation and accommodation are separate questions. Evacuation cover is not permission to fly to any hospital you choose. |
Already diagnosed? A new policy or an upgrade is not an automatic way to fund existing cancer. Disclose your history accurately and ask for written acceptance of the condition, including any exclusions or waiting periods. Check your existing cover before replacing it.
Ask about your treatment, not just “cancer cover”
Send the proposed plan and request answers specific to your case. These questions help you identify gaps before a deposit is due.
Country and facility
Is planned care covered in this country, at this exact hospital and department? Is out-of-network care reimbursable?
Diagnosis and treatment
Is my diagnosis covered under the underwriting terms? Are the named drug, indication, procedure and treatment setting eligible? Ask separately about off-label use, cell therapies and experimental treatment.
Limits and patient share
What annual or treatment limits remain? Which deductible, co-payment or co-insurance applies, and are there price or room-category limits?
Continuity of care
Does the decision include scans, pathology, outpatient medicines, complications and follow-up? What needs a fresh approval if the plan changes?
Keep the insurer’s answer with your policy documents. A general benefits summary cannot resolve every detail of an individual claim.
Approval and payment are different steps
Pre-authorisation is an insurer’s assessment before treatment. A guarantee of payment (GOP) is a payment undertaking sent to a provider, with stated conditions. Ask the insurer and hospital to confirm both the approved scope and the settlement arrangement.
Submit a defined plan
Ask the clinical team for the diagnosis, proposed treatment, dates and an itemised estimate. Submit the documents required by your insurer.
Read the written decision
Check the hospital name, eligible services, authorised amount, validity dates, exclusions and approval reference. Ask what changes must be reported.
Confirm the hospital can use it
Have the billing team confirm that it has received and accepted the payment arrangement. Establish any deposit and patient balance before admission.
| Payment route | What it means for your cash flow |
|---|---|
| Direct billing | The hospital bills the insurer or administrator for eligible care under the agreed arrangement. You may still owe a deductible, co-payment, exclusions or amounts above the authorised limit. |
| Pay and claim | You pay the provider, then submit the claim. Confirm eligibility, required documents, deadlines and reimbursement currency first; submitting a claim does not guarantee payment. |
| Self-pay | You fund the care directly. Request a written estimate, payment schedule, refund terms and a process for approving extra costs. |
Allow time for review: Allianz Care’s general process asks for submission at least five working days before treatment. Your plan may differ; this is not a guaranteed decision time. For an emergency, seek care and follow your policy’s emergency notification procedure.
Plan for the money insurance may not pay
Build a budget from the hospital’s written estimate and your insurer’s response. Keep unconfirmed reimbursement separate from funds already available.
Clinical costs
Ask for separate figures for consultation, tests, treatment, medicines, admission and follow-up. Identify items outside the approved scope.
Time away from home
Include flights, visas, accommodation, an accompanying person, local transport and interpretation. Ask what a longer stay or a return visit would cost.
Cash needed before reimbursement
Confirm deposit and payment deadlines, currency conversion, bank or card charges, refund timing and funds for unexpected care.
If there is a funding gap
Ask the hospital about a financial counsellor, payment schedule or assistance programme; check eligibility for overseas patients before relying on it. For any loan, compare the total repayment, interest, currency exposure and cancellation terms with an appropriately qualified adviser. Avoid making treatment plans depend on unconfirmed fundraising.
A clinical trial is not automatically a free treatment package
Ask the research team to separate study-funded procedures from routine care, complications and travel costs. Eligibility and support vary by study. NCI’s explanation is useful for understanding these categories, but US coverage rules do not establish entitlement in another country.
Explore clinical trialsUsing international insurance in China
Apply the same checks again to the exact mainland China facility. A global hospital network, a hospital’s insurer logo and an international department do not establish coverage for your treatment.
Match the hospital and department
Give your insurer the hospital’s Chinese and English names, address and billing entity. Ask whether the relevant campus, international department and treatment setting are included.
Identify who handles the approval
The insurer may use an assistance company or third-party administrator (TPA). Confirm who issues the authorisation or GOP and who the hospital contacts if the plan changes.
Separate treatment access from reimbursement
A therapy’s availability or approval in China does not establish insurance coverage. Submit the exact medicine, indication and proposed course for a case-specific decision.
Chinese social medical insurance and international commercial insurance are different systems. A hospital’s designation for local social insurance does not give every overseas visitor entitlement to it.
What hospital information can—and cannot—tell you
These are examples of published billing information, not a ranking, a complete directory or confirmation of a Chirona partnership. Choose clinical expertise first, then verify the payment route for your own policy.
Jiahui Health, Shanghai
Jiahui’s official insurance page states that it offers direct billing with major insurers and updates its partner list. Its cancer centre also publishes insurance payment information.
Confirm for your caseAsk whether your exact plan, treating facility and proposed oncology treatment qualify, and which approvals and patient payments are needed.
United Family Healthcare
UFH publishes facility-specific direct-billing lists. Its general guidance says patients remain responsible for their applicable co-payment or deductible and that direct billing uses primary insurance.
Confirm for your caseConfirm the city, hospital and oncology service with its billing team. A network-wide statement is not confirmation for every facility or treatment.
Considering another hospital?
Ask its international patient office or billing team whether it can accept your insurer’s GOP, arrange case-specific settlement or provide documentation for a reimbursement claim. Until both sides confirm, budget for a deposit or self-payment; do not infer acceptance from another hospital’s arrangements.
Your pre-travel confirmation file
Use this list for any destination, including China. Tick an item when you have the answer or document—not simply when you have asked for it.
This checklist resets when you reload or leave the page. Do not enter policy numbers or medical records here. For China, confirm whether the insurer needs the original medical invoice or fapiao, the itemised charge list and translated discharge records; requirements differ.
Prepare your medical documentsCommon questions
Can I buy insurance after a cancer diagnosis?
You can ask an insurer about available terms, but a new policy may exclude the existing cancer or impose restrictions. Group and individual arrangements can differ. Do not cancel current cover or assume a new policy funds the proposed treatment without written confirmation.
Does direct billing mean I pay nothing?
No. Direct billing describes how an eligible bill is settled. Your policy and the approval may still leave a deductible, co-payment, excluded services or costs above limits for you to pay.
Will international insurance cover CAR-T treatment in China?
There is no universal answer. Ask for a written decision on the named product, diagnosis, indication, hospital and full treatment course, including monitoring and complications. A drug’s regulatory approval and the hospital’s insurer list do not establish coverage.
Can I claim if the hospital is not in my insurer’s network?
Possibly, if your policy allows it and any required approval is obtained. Ask about reduced benefits, price limits, documents and upfront payment before proceeding. The hospital’s ability to issue an invoice alone does not make the claim eligible.
What should I do if approval is refused?
Request the reason and the relevant policy clause in writing. Ask about the review or appeal process, missing evidence and deadlines. Discuss clinically appropriate alternatives and timing with your treating team, and obtain a self-pay estimate if needed.
Put the treatment plan and the payment plan together.
Discuss your diagnosis, proposed destination and practical questions with Chirona. Your insurer decides coverage; the treating hospital confirms clinical care and billing arrangements.
