MRG003 for nasopharyngeal cancer: eligibility, previous treatment and access

For adults with recurrent or metastatic nasopharyngeal cancer after chemotherapy and immunotherapy have failed, MRG003 (becotatug vedotin) is an ADC option to assess. Eligibility depends on failure of at least two systemic chemotherapy lines and PD-1/PD-L1 treatment. Chirona arranges partner-hospital review and coordinates drug supply, infusions, follow-up and costs.

What is MRG003, and how does it differ from an EGFR pill?

MRG003 and becotatug vedotin are the same medicine, marketed in China as Meiyouheng. This antibody–drug conjugate (ADC) links an EGFR-targeting antibody to the anticancer payload MMAE and is given intravenously.

It is not an EGFR-targeted pill such as osimertinib. An EGFR mutation does not by itself establish eligibility, and approval does not cover every EGFR-positive cancer. Start with the nasopharyngeal cancer diagnosis, disease setting and treatment history; the specialist then determines tests and options.

Which nasopharyngeal cancer patients fit the approved use?

China granted conditional approval in October 2025 for adults meeting the criteria below. These requirements apply together; repeated chemotherapy visits or an EGFR-positive result alone do not establish eligibility.

Four points to check before assessment
CheckApproved-use requirementWhat to provide
Age and diagnosisAn adult with nasopharyngeal carcinoma.Age and pathology report; a general head-and-neck cancer diagnosis is not enough.
Current diseaseRecurrent or metastatic disease.Recent scans and recurrence/metastasis records, including previous radiotherapy fields.
Chemotherapy historyFailure of at least two systemic chemotherapy lines.Drugs, dates, response and reason for stopping each line; two cycles do not mean two lines.
Immunotherapy historyFailure of PD-1/PD-L1 inhibitor treatment. The drug, combination/maintenance history and progression records; a PD-L1 test cannot replace treatment history.

Can treatment be arranged through the approved pathway in China?

Eligible patients can be assessed through an approved-use pathway. MRG003 has been approved and commercialized in China; access is not limited to clinical trials. Chirona arranges specialist review at partner hospitals, then coordinates hospital drug supply, infusion visits, tests and costs once the clinician agrees a plan. The hospital and available dates are confirmed before travel.

What about other head-and-neck cancers or immunotherapy combinations?

Other head-and-neck squamous cancers and combinations such as MRG003 plus pucotenlimab need separate assessment of the relevant research or prescribing basis. The nasopharyngeal monotherapy criteria do not automatically cover them. Reported trial progress and US FDA development designations are not marketing approvals; supply and payment arrangements in other countries require separate confirmation.

What benefit has been shown compared with later-line chemotherapy?

The randomized Magic-M001 trial enrolled 173 patients with heavily pretreated recurrent/metastatic nasopharyngeal cancer and compared MRG003 with capecitabine or docetaxel. Results were presented at ASCO 2025 and published in Annals of Oncology in 2026.

Results and what they mean
MeasureMRG003 versus chemotherapyHow to interpret it
Tumor response30.2% vs 11.5%More patients met the tumor-shrinkage criteria; benefit was not universal.
Median progression-free survival5.8 versus 2.8 monthsProgression was delayed in the study population; a median is not an individual treatment deadline.

Has longer overall survival been proven?

Overall survival was still immature, and the main analysis did not show a statistically significant difference. The evidence supports improved response and progression-free survival, but does not establish a definitive survival extension. Side effects, recovery and other available options also matter.

Which previous treatments count, and which records are needed?

Arrange your treatment history by date; you do not need to classify every line yourself. We help organize the records for a specialist to confirm eligibility and compare local treatments or other medicines.

Chemotherapy and immunotherapy: four details that matter

  • Name each full regimen: include drugs, start/end dates and any immunotherapy partner. Two chemotherapy drugs within one combination do not equal two treatment lines.
  • Separate cycles from treatment lines: repeated doses of one regimen generally remain one line. Switching after progression differs from a dose adjustment. The clinician reviews how induction chemotherapy, chemoradiotherapy and maintenance fit into the full history.
  • Document PD-1/PD-L1 treatment: include combination and maintenance use, scans showing progression or the clinician’s assessment of treatment failure. Record toxicity-related discontinuation separately; it is not automatically the same as progression.
  • Include radiotherapy and current disease records: provide treatment fields, a dose summary, recent scans and symptoms. For a local recurrence, discuss whether local treatment has a role alongside systemic options.

What if I have had another ADC or am considering iza-bren?

MRG003 and iza-bren use different target designs and payloads. There is no established direct head-to-head evidence for ranking them, and response rates from separate trials should not be compared as if they were. Provide previous ADC names, response, reasons for stopping, blood-count recovery and any numbness to assess the evidence and tolerability of another ADC.

How is treatment given, and what needs monitoring?

MRG003 is infused intravenously in hospital. The treating clinician sets the dose, cycle and need for admission using current prescribing information, your health and side effects. The plan should include infusions, blood tests, scans and a contact route between visits.

Report specific changes, not just feeling unwell

  • Record nerve symptoms before starting: numbness, tingling or weakness, and any effect on walking, balance or fastening buttons. Report existing symptoms rather than hiding them or deciding on your own that treatment is impossible.
  • Check blood counts and organ function before dosing: low counts, infection or unresolved toxicity may require a delay or adjustment, decided by the treating clinician.
  • Report changes between visits: worsening numbness, fever, marked weakness, bleeding or reduced intake should not wait until the next infusion. Tell staff immediately about a sudden rash, chest tightness or breathing difficulty during an infusion.
  • Reassessment guides continuation: scans, symptoms and tolerability matter more than completing a preset number of doses. Severe breathlessness, substantial bleeding or altered awareness requires emergency care.

What could treatment cost, and what needs separate confirmation?

US$28,000–76,000

What’s included

This estimate covers total medical treatment costs, including medication, specialist consultations and assessment, pretreatment tests, routine laboratory and imaging follow-up, treatment administration and nursing, necessary hospitalization and monitoring, and routine supportive care. Travel and accommodation are not included.

Your individual treatment costs

Ongoing treatment does not have one fixed complete course for everyone. Dose, infusion visits, reassessment, admission and supportive care affect the total, which may exceed this range. We help obtain itemized estimates for medication, infusions, tests and expected admission, and clarify charges for complications such as serious infection, additional treatment, coordination, travel and accommodation.

How does Chirona turn the assessment into treatment arrangements?

Send your existing records to start. We arrange specialist review at partner hospitals and coordinate the treating hospital, medication plan, costs and follow-up. You can contact us before every document is complete.

1. Organize diagnosis, treatment history and current health

Provide pathology, recent scans, line-by-line chemotherapy and PD-1/PD-L1 history, radiotherapy summary, recent tests and current medicines. Note numbness, infections, serious side effects and changes in daily activity. We identify missing records for review.

2. Confirm the plan, drug supply and first visit

After the specialist reviews eligibility and alternatives, we coordinate appointments, drug supply, pre-infusion tests and itemized costs. Before travel, confirm available services and dates, plus any assessments that must take place at the hospital.

3. Arrange monitoring between visits and care at home

Confirm where blood tests, scans and urgent assessment for fever or nerve symptoms will take place, and when the next cycle will be reviewed. We help prepare records for your home clinicians. Continuing infusions at home requires separate local supply and prescribing arrangements.
