Renji Hospital Performs Robot-Assisted Partial Resection of a Tumor in a Transplanted Kidney

Recently, Pan Jiahua, director of the Urology Department of the Affiliated Renji Hospital, assembled a multidisciplinary (MDT) expert team including Professor Huang Yiran, an authoritative expert in the field of urological tumors and chief physician of the Department of Urology, Professor Xue Wei, chief physician, and Huang Jiwei, chief physician, to successfully remove the tumor from the transplanted kidney of Mr. Li, 56, from Henan.

Mr. Li received a kidney transplant five years ago due to kidney failure. Recently, a tumor grew inside the transplanted kidney. In view of the risks such as the high degree of adhesion in the surgical area, variations in vascular anatomy, and immunosuppression, there is still a lack of expert consensus on treatment options, and traditional radical nephrectomy is mostly recommended in the industry. Pan Jiahua's team comprehensively assessed the condition, repeatedly deduced the surgical path, and finally formulated a precise surgical plan for "robot-assisted partial resection of renal tumors in kidney transplantation". They also carefully prepared a backup plan for open surgery and decided to save Mr. Li's precious kidney.

The surgery was performed by Huang Jiwei, who operated the Da Vinci robot to remove Mr. Li's tumor. The da Vinci robotic system shows its talents: its 3D high-definition field of view magnifies the surgical area by more than 10 times, and its 540-degree flexibly rotating robotic arm accurately separates and completely removes tumors in a small space. The team accurately blocked the iliac blood vessels in only 15 minutes, effectively controlled bleeding, preserved normal kidney tissue to the greatest extent, and ensured that the blood supply of the transplanted kidney was not affected. The entire operation is like a high-precision "mine clearance", and every step is extremely accurate. Mr. Li recovered smoothly after the operation, his creatinine value was stable at around 100 μmol/L, his renal function was not affected, and he was pathologically diagnosed as clear cell renal cell carcinoma.

According to Huang Jiwei, cases of renal tumors occurring in transplanted kidneys are rarely reported. According to relevant foreign data, the incidence of tumors in transplanted kidneys is only 0.2%-0.4%, and minimally invasive surgical treatment of such patients still faces many challenges. Therefore, the success of this difficult operation fully reflects the advantages of the multidisciplinary collaboration (MDT) model, especially the close cooperation of the urology kidney transplant team. In the future, the affiliated Renji Hospital will continue to promote the development of minimally invasive treatment technology for renal tumors and provide more patients with accurate, safe and efficient diagnosis and treatment services.
