Brain metastases

Cancer that spreads to the brain remains the original cancer type. Brain metastases are different from a primary glioma. Local brain treatment and treatment for cancer elsewhere need a coordinated plan.

Assess the brain and the original cancer

Contrast-enhanced brain MRI helps define the number, size and location of lesions. Symptoms, surrounding swelling, disease outside the brain and the original tumor’s biomarkers all affect the plan. New seizures or sudden neurologic changes need urgent clinical assessment.

Coordinate local and systemic options

Surgery, stereotactic radiation and whole-brain radiation have different roles. Selected targeted medicines or immunotherapies may also have intracranial activity. Treatment sequencing depends on symptoms, lesion characteristics and cancer type.

Protect function and plan follow-up

Ask about cognition, mobility, symptom medicines and the timing of follow-up MRI. Changes after radiation can reflect treatment effects as well as cancer activity and may require specialist interpretation.

Advanced therapies & clinical trials

When reviewing a new option, separate the evidence for the treatment from the practical route to receive it. An approval in one country does not establish availability or eligibility elsewhere.

Advanced therapies & clinical trials
OptionWhat to verify
Approved treatmentExact indication, age group, treatment line, biomarker requirements and the current local prescribing information.
Clinical trialRecruiting site, inclusion and exclusion criteria, prior therapies, travel and follow-up requirements. Participation is not guaranteed.
Off-label or special accessClinical rationale, local legal route, risks, costs and the treating institution’s ability to provide follow-up.

Preparing for a second opinion

A second opinion is especially useful when pathology is uncertain, several treatment paths are possible, disease has returned or a new therapy is being considered. Coordinate timing with the treating team so necessary care is not delayed.

  • Pathology report and access to tissue slides or blocks
  • Imaging reports and the actual scans, with dates
  • Molecular and biomarker reports, including the test method
  • Treatment names, dates, response and major side effects
  • Current medicines, other conditions and the questions you want answered
